2. Notarized Minor and Parental Consent Form - This form must be
printed and notarized prior to your appointment.
a. Print out the form from our website.
b. The parent/guardian will be required to provide a copy of a government-issued photo ID
that will be copied at the appointment time, and written documentation showing a lawful
parent/guardian relationship to the minor patient.

. The minor patient must ALSO have a photo ID. If you do not have a government-issued ID,
you may use another form of identification with a photo, such as a copy of a yearbook
page.
4. Government-issued ID of parent or guardian and photo ID of minor patient.
a. State issued ID (driver’s license, etc.)

c. Passport
d. Military ID
e. Yearbook page with photo (minor only, if no other ID available)
5. Legal papers linking the minor to the parent or legal guardian.

The State of Oklahoma requires all unemancipated minors seeking an abortion to obtain
notarized written consent of the minor and one parent or legal guardian of the minor.
Both the minor and parent/legal guardian must sign an informed consent. The
following is a list of documentation the minor and the parent/legal guardian need
to bring with them to the appointment.
1. Informed Consent - The State of Oklahoma requires that state-mandated
information must be provided at least 72 hours before the scheduled appointment.
You can begin the consent process in the following ways:
NOTE: If the parent’s name differs from the name that appears on the birth certificate
(for example, parent has remarried) documentation must be submitted verifying the
parent’s connection to the minor. Acceptable documentation would include:
a) Divorce papers showing a name change
b) Custody agreement
c) Guardianship papers
d) Marriage certificate showing name change of the parent
Other Important & Required Information
Parent and/or legal guardian will need to be present during the appointment.
If you are a minor and do not or cannot obtain consent from a parent or legal guardian,
contact our health center to go over information on obtaining a judicial bypass, which is
a court order allowing minors to receive an abortion. Please call 405-603-4884
(Oklahoma City) or 918-858-4661 (Tulsa) and one of our representatives is available to
help. An Oklahoma District Court judge may waive the parental consent requirement, if
necessary and appropriate, with a court-appointed (free) attorney, per 63 OS §1-740.3.
If you are eligible for financial assistance, you may be asked to bring documentation
with you to the health center.
Missouri patients only: If you are a minor from Missouri, the "Notarized Attestation of
Second Parent Notice" form will need to be completed before your appointment.
